| Details: The Well Child Clinic offers doctor, dental and vision clinics. Clinics are on Tuesdays. For a copy of the current calendar and more information, call 260 665 2403 or click on the "Contact Us" link below and leave us an email.. Office hours are Tuesday - Thursday from 8:30am to 12:30pm. Name, phone number, and address of those interested can be left on the answering machine, which is available 24 hours a day. |
| Special Programs: FMAP Grant - Offers assistance with medical bills. May cover partial payments for office visits and medicines. Can help when you have a high deductbile on your employer's family health insurance. See office staff to determine eligiblity. Medicaid recipients are not eligible. |
| The Well Child Clinic is located in the Steuben Community Center Building (the old Angola High School). It has been in existence for over 25 years. The clinic serves all children ages birth through age 7 and pregnant mothers. It is a non-profit organizations that is funded only through donations and grants. |
