Dear Brother David. here is your template. Be careful about deleting any of the files in your File cabinet. NEVER 'Delete' your 'index file.'

Below I also added a Table for you to put your links to your work. Study it's structure well and be carful when you replace the links I have with your own.

I have called this file 'clinic.htm' but you can rename it. No problem.
