Health care outreach and case management services provided to recipients of Aid to Families with Dependent Children on Medicaid, ages birth to 21 and to participants in Baby Care/Kids Care program.

Provides vouchers for supplemental foods and nutritional education for pregnant or breastfeeding women, infants and children up to five years of age. Applicants must qualify for service.

Pueblo Developmental Evaluation Clinic (PDEC) 583-4360
The Pueblo Developmental Evaluation Clinic is a multidisciplinary, interdisciplinary, interagency, community-based, coordinated effort to provide comprehensive quality diagnostic evaluations. The clinic makes recommendations for services for children who have or are suspected of having complex delays related to a medical health concern. Age eligibility is: birth through 13 years of age; if on Medicaid birth to 21 years of age. The evaluation team includes: a developmental pediatrician, physical therapists, speech language specialists, audiologists, social workers, educators, psychologists, public health nurses, and a clinic coordinator. International Travel Clinic 583-4363
This program provides education regarding vaccine preventable diseases and health related travel tips to individuals traveling outside of the United States. This includes a health history assessment and administration of required or recommended vaccinations. Appointments are required and it is recommended that you schedule an appointment three months prior to the planned trip. Payment is required at the time of service.

La Familia Fuerte/Strong Families 583-4562
An outreach and home visitation program that provides services to families with children under age three in the Fountain/Bradford neighborhoods. This project helps families receive support and community services through a home visitor, public health nurse and outreach coordinator. This is a joint venture of Bradford Elementary School, Parkview Episcopal Medical Center, St. Mary Corwin Regional Medical Center, Pueblo City-County Health Department, School District #60 and numerous other caring community agencies.
